What causes a urinary tract infection?


A UTI develops when bacteria, most commonly Escherichia coli (E. coli), enter the urinary tract and begin to multiply. While E. coli is a normal inhabitant of the digestive tract, it can sometimes spread to the urethra and bladder, leading to infection. How is a UTI diagnosed during a visit?

We will ask about common UTI symptoms such as burning during urination, increased urgency or frequency, lower abdominal pain, and any previous history of UTIs.

After discussing your symptoms, a focused physical examination may be performed, especially of your abdomen and lower back, to check for tenderness or signs of a more serious infection. The most important step in confirming a UTI is a urine test. You will be asked to provide a clean-catch urine sample—this means cleaning the genital area and collecting midstream urine in a sterile cup to minimize contamination.

Your urine sample will first be analyzed onsite with a dipstick test, which can quickly detect signs of infection, such as the presence of white blood cells, red blood cells, and nitrites. If these results suggest a UTI, we may send your urine to a laboratory for a urine culture. This test helps identify the specific bacteria causing your infection and determines which antibiotic will work best for you.

What happens if a UTI is left untreated?


If left untreated, a UTI can lead to serious health complications. Early symptoms—such as frequent or urgent urination, pain or burning when urinating, and abdominal discomfort—can quickly worsen without proper care.

If a UTI is not addressed, the infection can spread from the bladder and urethra (lower urinary tract) to the kidneys (upper urinary tract), resulting in a condition called pyelonephritis. This more serious kidney infection may cause high fever, chills, back or flank pain, nausea, and vomiting. In rare cases, the infection can even enter the bloodstream, leading to a life-threatening condition called urosepsis, which requires emergency care.
